The output from the hooks is in the form of classads, so the fetch hook would be the classad to run and the prepare hook the attributes to modify (if any). One entry per line, and appropriate quotes around values if needed.

If by "expected" you mean the data sent to the hooks via stdin, then yes. What is displayed in condor_q -l is what will be sent to the hook.
